Renew Active™, a fitness program to help you stay fit, stay focused, and stay you, is administered through UnitedHealthcare® Medicare plans. To confirm eligibility, we need a confirmation code. You can locate your code by signing into your UnitedHealthcare® account or calling the number on the back of your insurance card.
